Who designed the "broken image" icons?

I'm Hagan Heller (or I was, until I married and change my name to Hagan Rivers). I was the UI designer at Netscape for versions 0.n - 4.n. All of the icon work through Netscape 3.0, including the broken image icon, the throbbing in, the Mosaic tiles, and the final Netscape animation were produced by the very talented Marsh Chamberlin, who was a contractor working for the company. I hired him. He still works out of his web site Dataglyph.com. (In Netscape 4.0 we had a full time employee named Shuang Li who was responsible for all the icons and artwork).

From the Reply by Hagan Rivers in Quora discussion Who designed the "broken image" icons?

Before the first version of Netscape, there wasn't a specific icon that meant "broken image", but there were placeholder images used for missing images in the product. Marsh actually produced icons for broken image, missing image (two different things), and also the icons for the individual file types that might show up in a file listing.

In the same thread you can also find pictures of each round of the design work.

Looking for a original release of the Mozilla source code I stumbled across a browsable git instance of Mozilla's formerly used version control starting 03/1998 (Danger. Here be monsters.). Commited on 27.7.2006 there is a copy of broken.gif.

Based on the bitmap I created vector versions. To fit the standardised sticker options I created a squarish variant with enough bleed to compensate the rather large variation with the current printing option.
